Please do not turn in any documentation to Coaches or Front Office Staff. Either submit the documents online or turn directly into athletic training staff for assistance.

All physicals must be submitted on an official UIL physical form to be accepted, and must be dated on or after April 1st, 2024 for the '24-25 school year. For a blank copy of the form, click the link at the top of the page, or your child can pick one up in the Athletic Training Office G141.
